Check your eligibility

Complete a short questionnaire about your current situation — when you were diagnosed, who by, and what support you have now. It takes a few minutes and tells us whether we’re able to help.

We review your records

With your consent, we obtain and review your existing clinical records. A proper handover depends on a full picture, so we ask for it before we take anything on.

Ongoing reviews

Where clinically appropriate, we provide continued clinical support with regular reviews at agreed intervals — so your care has a rhythm rather than a queue.

Questions people ask first

I was diagnosed elsewhere. Can you take over my care?

Often, yes. We’ll ask for your existing clinical records first and review them properly. Whether we can take your care on, and what that looks like, depends on that review — we’ll be straight with you either way.

Do I need a new assessment?

Not always. If you have a documented diagnosis and records we can obtain, a full reassessment often isn’t needed. If your records aren’t available or the picture isn’t clear, we’ll tell you what’s required before you commit to anything.

My shared care request was refused. Can you help?

This is one of the most common reasons people come to us. Where a GP has declined shared care, we can provide continued clinical support directly, so your care doesn’t stall while that’s unresolved.

Do I need a referral from my GP?

No. You can come to us directly. We will, with your consent, request your records — but you don’t need a referral to enquire.

Who provides the care?

Your care is provided by a Nurse Independent Prescriber. Focus Gently is registered with the Care Quality Commission, the independent regulator of health and social care in England.

Is everything done remotely?

Yes. Enquiries, reviews and ongoing support are all carried out remotely, so you can be seen from home wherever you are in the UK.
